Help with MS4 Cup or S4 Challenge Carbon Heel plates.

Anyone got an MS4 Cup or Challenge bike or similar fitted with the DP carbon heel plates for the DP rearsets fitted to these bikes?

I'd like to know how the spacers fit and what the 5th bolt is for. If you could supply close up photos of how the heel plates are mounted that would be useful since there is not a lot on internet.

Thanks
