Banks already have a department set up to deal with PPI. Just call them and tell them you think you've been missold PPI and they'll do the rest. I got just over £4k myself with no hassle whatsoever.

I did it with zero memory of my loans from 20 years ago.
I just phoned the my bank and got them to send me paper statements going back as far as possible. The went back to 1999.
Then I went through and made notes on the the direct debits that left my account. Noting the name, value, reference and the time period the payments left my account.
Then I Googled the company name for a ppi complains address and sent them a letter saying they had misplaced sold to me.... A couple of them had online forms to fill in so you didn't even have to send them a letter.
When you send that letter they have 6 weeks to investigate and make a decision.
I got my decision 4 days early and the cash the following day straight into my bank.....

I'll find a copy of the letter I sent and pin it on here..

It's well worth it. I only expected 500 quid.... £3411 came in.... My mate blindly did the same and got 12k back last week

Your name and addresses

Lloyds Bank
PPI Customer Services
BX1 1LT

FAO: Complaints Manager

Dear Sir/Madam,

Re: 100007040615 / 100023028021 / 100113956313 / 100119907830

I purchased the above policies from you between 1998 and 2013 in connection with the loan referenced above I believe I was mis-sold these policies for the reasons given below, and wish you to investigate my complaint according to your normal complaint procedures.

I was not told that the insurance was optional.

I was led to believe my application for credit would be rejected if I did not take out the insurance.

The policy exclusions were not explained to me either before, or at the time, I took out the insurance. I was therefore unable to make an informed decision as to whether this insurance was appropriate for me.

If my complaint is upheld I require a refund of all premiums paid to the policy, plus any additional interest charged to me because of the PPI.

I would also like to claim statutory compensation in view of the fact that I have been deprived of the use of this money.

For reference my address at the time of these agreements was as below.
Old addresses listed here

Please now investigate my complaint within the eight weeks allowed to you.

Yours faithfully,
